DVX Warranty & Returns


This form is to be used for making warranty claims only for DVX Eyewear. If you purchased glasses directly from Wiley X and want to return them for a refund please contact customer service at (800) 776-7842.

By completing this Return Authorization Request you acknowledge that you have read and understood the terms and conditions as set forth in Wiley X, Inc. product warranties. Please read the warranty policies by clicking on the links below.

To begin the warranty/repair evaluation process, please fill out this online warranty repair form. Please be sure to upload photos of the damages claimed. Once the return request is received and approved, a Wiley X Customer Service Representative will contact you with a Return Authorization (RA) reference number. After receiving the RA, please print the completed online form with RA# and mail it in with the item that you are returning using a traceable shipping method as Wiley X is not responsible for lost or damaged packages. Please allow 7–10 business days from receipt of product to process your claim. Return Authorizations are valid for 90 days from the date they are requested. Note: If multiple items are being returned an RA number must be completed for each item. Items can be shipped/returned together.

Please note that during the RA checkout you will not be able to place any purchase items into your cart until you complete the RA checkout. You can end the RA checkout process at any time by going to the view cart page and emptying your cart.

Return Shipping Information

Please send all warranty items within 90 days of the Authorization Request date using a traceable shipping method as Wiley X is not responsible for lost or damaged packages.  Please include the RA# on the outside of the package.

All DVX Eyewear is warranted to the original purchaser for one year against manufacturing defect in materials and workmanship. If a defect is observed, a claim should be made immediately. This warranty does not apply to normal wear, including but not limited to scratched lenses, accidental damage, misuse and other issues determined by DVX Eyewear not to be a manufacturer’s defect. For Canadian residents, the original purchaser must include a shipping, handling and processing fee of $27.00USD (sorry, no checks) with any warranty claim. Tax where applicable will also accompany this processing fee.

Upon receiving returned eyewear that DVX Eyewear determines to be defective, eyewear will be repaired or replaced at DVX’s option. Non-warranty repair services are available as it pertains to lenses or frames that are not covered under terms of warranty. DVX will email customers with their options and applicable costs and will make repairs upon acceptance. Items not covered under terms of warranty which are also not repairable will be returned to the customer or destroyed at customer’s option.  For Canadian residents, the $27.00USD (plus applicable taxes) processing fee will not be charged.

Prescription orders processed through DVX Eyewear will have a one (1) year warranty from the date of purchase on frames, lenses, and coatings deemed by DVX to have manufacturing defects. Prescriptions filled by non-authorized labs will void all DVX frame and lens warranties. Prescription frame and lens warranties must be handled through the local retailer from which they were originally purchased. Frames including prescription lenses which are sent directly to DVX Eyewear without prior authorization will be returned to sender.
